Following the UK’s departure of the EU, we need you to confirm a UK correspondence address by completing and returning the form below.

Please fully complete and sign the form and then either:
- Post it to FAMILY BUILDING SOCIETY, EBBISHAM HOUSE, 30 CHURCH STREET, EPSOM, KT17 4NL, ENGLAND
- Upload it via our secure upload.
